Accountability diffusion

Definition

Responsibility spreads across teams, vendors, and incentives until no one can intervene. Accountability diffusion erodes design authority and leaves harm without a decisive owner. If no one can unilaterally halt the system or reverse the decision behind the harm, accountability is diffused.

Diagnostic

Tests that show it is in place

  • Escalation maps name a single owner with authority to halt or repair.
  • Incident reviews show each handoff reaching a clear decision point.

Hypothetical example

  • A support ticket bounces between a vendor and a hospital compliance team, and no one has authority to halt the rollout.
