Definition
When rigid policy checklists replace judgment, causing teams to follow rules while harm worsens. It occurs when the authority to change the rules is weak and decisions are hard to challenge, leaving no path to halt or repair.
Tests that show it is in place
- Teams that meet checklist requirements can also halt the system and reverse the decisions that caused the harm.
- Procedures give escalations discretionary authority or an override lane, so they do not stall.
